Fred Elbert Johnson, Sr., 73 of Lake City died early Monday morning, July 12, 2004 at the Veteran's Affairs Medical Center in Lake City following an extended illness.
A memorial service for Mr. Johnson will be conducted on Wednesday, July 14, 2004 at 2:00 P.M. at the chapel of Gateway~Forest Lawn Funeral Home with Rev. Ivan K. Clements, pastor of Calvary Baptist Church officiating.
The family is requesting that no flowers be sent for the services. Please send memorial donations to the Calvary Baptist Church Building Fund at P.O. Box 1353, Lake City, Columbia, Florida 32056 in lieu of flowers.
